Login & Register Forms reset-code brute force permits account takeover
Login & Register Forms before 4.0.2 keys both password-reset verification codes and per-source attempt counters on client-controlled data rather than a server-derived identity. When verification-code reset mode is enabled, an unauthenticated attacker can repeatedly change that data to reset the attempt limit, brute-force a target's code and take over any account, including an Administrator account.

Update to 4.0.2 or later immediately. Until updated, disable verification-code password resets or deactivate the plugin. Review password-reset activity and privileged accounts for brute-force patterns or unauthorized changes, invalidate sessions and reset credentials where compromise is suspected.
A safe version is available, so updating to that version or later is the preferred remediation. If an immediate update is not practical, consider a targeted application patch or temporarily restricting the affected functionality.

Technical description
The Login & Register Forms WordPress plugin before 4.0.2 does not enforce its password reset attempt limit against a server-derived value, keying both the verification code and the per-source attempt counter on client-controlled data, allowing unauthenticated attackers to reset the limit at will and brute-force the code to take over any account, including administrators, when the verification-code reset mode is enabled.
